To complete the proposal process, you’ll need to have the following information handy:
For Business Owners Policy:
- Copy of your current insurance policy.
- Information concerning each business location including:
- Location address.
- Approximate age of building.
- Construction of building.
- Approximate area your business occupies.
- Any additional occupants in the same building.
- Fire and Theft protection information.
- Legal business name and business type.
- Detailed description of business operations.
- Gross Annual sales/receipts and payroll.
- Number of years in business or owners' experience.
- Federal Employee Identification Number (FEIN).
- Prior loss information (3 years).
For Commercial Auto:
- Street address of each of your properties.
- Copy of your current insurance policy.
- Copies of your current loss runs or detailed loss information for up to the last five years. (These can be obtained from your current insurance carrier or your agency representative.)
- List of vehicles you own, including:
- Vehicle Identification Number (this may be on your current policy or the vehicle registration).
- Year, make and model of vehicle.
- List of drivers of work-related vehicles.
- Drivers’ license numbers.
- Drivers’ dates of birth.
For Umbrella:
- Copy of your current insurance policy.
For Workers' Compensation:
- Street address of each of your properties.
- Copy of your current insurance policies.
- Payroll records indicating:
- Total number of employees at each location.
- Total payroll expense for each property.
- Payroll by source of expense.
