Automatic Draft Form
Take the worry out of remembering when to make your mortgage payment. Sign up online or send us a completed Automatic Draft Form. The mailing address and fax number are included in the downloadable form.
Borrower Information Release Form
Authorize another party to obtain information regarding your mortgage account with the Borrower Release Form. Please note that certain transactions cannot be processed unless requested by the borrower of record.
Credit Dispute Form
Should you find a discrepancy between your records and what is reflected on your credit report, please complete this Credit Dispute Form and return it to us at the address below along with any documentation relevant to the dispute.

W-9 and W-8BEN
The IRS Forms W-9 and W-8BEN are for obtaining your correct taxpayer identification number (TIN) to report, for example, income paid to you, real estate transactions, mortgage interest you paid, acquisition or abandonment of secured property, cancellation of debt or contributions you made to an IRA.

Loan Servicing Fee Schedule: Fees will be charged as follows or as permitted by applicable law.

| Conversion | $100 | Fee authorized in agreement to convert all or portion of Credit Line Account variable rate balance to a fixed rate with a fixed term |
| Card Rush | $35 maximum | Fee to expedite delivery of a Credit Card for Credit Line Account at borrower's request |
| Principal Reduction Modification Fee | $275 | Fee for process of a Principal Reduction modification & for overnight of documents. A FedEx return envelope will be include to return documents for processing |
| Escrow Release Fee | 0.25% of the current unpaid principal balance | Fee charged if borrower’s request to discontinue the escrow of property taxes and hazard insurance is approved by Regions |
